Responsibilities include but are not limited to:

•  Instruct patients on diabetesself management skills in both individual and group settings
• Develop a care plan for patientswith diabetes (Type 1, 2, GDM, Pre-Diabetes)
• Teach administration of insulineffectively, including titration and management as per medical directives
• Assess manage patients oninsulin pump therapy, as per medical directives 
• Perform patient follow ups inperson, by e-mail or by telephone
• Contribute to the development ofpatient handouts, as required
• Stay current and aware ofopportunities to implement new, evidenced-based methods of patient assessmentand treatment
• Participation in ClinicalResearch projects, coordinating patient care including reviewing consentinformation , dispensing medications, using clinical nursing skills, collectingdata
